By scheduling or receiving services from VitalLink Diagnostics, you agree to the following terms:

Service Scope

​

VitalLink Diagnostics provides professional mobile specimen collection services. Laboratory testing and analysis are performed by an independent accredited laboratory.

Payment

• Full payment is required prior to specimen collection.

• Base service pricing covers specimen collection and laboratory processing only.

• Travel fees are calculated separately and added to the base service price at the time of booking.

• Once specimens are submitted to the laboratory, services are non-refundable.

Travel Fees

​

Travel fees are determined based on distance from our service location and will be disclosed prior to appointment confirmation.

Identification Requirements

All participants must present valid, government-issued photo identification at the time of collection.

​

Laboratory Processing

​

Results are issued by the accredited laboratory. VitalLink Diagnostics does not alter, interpret, or modify laboratory findings.

 

Limitation of Liability

 

VitalLink Diagnostics is responsible for proper specimen collection procedures. Laboratory analysis and result reporting are performed by an independent laboratory.
